Why providers feel this now

However CQC’s assessment method evolves, the constant is the evidence behind safe, effective, caring, responsive and well-led care: dated, signed and retrievable when it is requested. The rating is public and the consequences matter. For a small provider without a dedicated compliance lead, ‘show me your evidence’ is the part that hurts. CQC is splitting the single assessment framework into sector-specific frameworks by the end of 2026, with the quality statements reworked underneath. Evidence behind every answer

Whatever the current assessment format, CQC’s judgement is built on the same five questions (safe, effective, caring, responsive, well-led) and the evidence behind them. The anxiety for small providers is concrete: knowing which evidence is expected, and having it dated, signed and retrievable on the day. Ingleby Care Home met three of its four previous breaches in 2025 and stayed at Requires Improvement because of the fourth. CQC found audits without documented evidence, action plans without named owners or target dates, actions 'validated' the same day as the audit, and no one person in control of quality assurance.
